It looks like he had some kind of large cut in his armpit. Surgery, injury. That is way deep and far too large to be a cyst or a boil. If you can get the puss out the best thing to do is boil that out with peroxide and put neosporin on it. Everybody runs to the doctor for an antibiotic. I won't take them unless it's absolutely necessary
